Medical missions in Eritrea: Opportunities and challenges for sustainable cooperation

In March 2025, three members of met4a e.V. – Prof. Dr. Kaulhausen, Prof. Dr. Gnoth and Dr. Beurer – traveled to Asmara, Eritrea, to lay the foundations for a medical partnership with the Edaga Hamus Hospital. The aim of the one-week trip was to evaluate medical cooperation, provide operational support and impart theoretical knowledge. At the same time, a project proposal including a “Memorandum of Understanding” (MoU) was prepared in order to put the cooperation on a binding basis. Goal of the trip: Build partnership and share knowledge.

Challenges in arrival and accommodation
The journey was fraught with obstacles: Some of the luggage was left behind in Istanbul and arrived late. Nevertheless, work was able to begin as planned on March 24. Accommodation at the Sunshine Hotel proved to be adequate, but the high costs are a recurring topic of discussion.

Surgical procedures and medical cooperation
In close consultation with the local doctors Dr. Kifleyesus and Dr. Dawit, numerous procedures were performed – including operations for ovarian cysts, fibroadenomas, recurrences of carcinomas and hysteroscopies. The focus was also on diagnostics for fertility treatment, e.g. by tubal flushing and contrast sonography.

Potential and limitations of the medical infrastructure
It became clear that the operative capacities depend heavily on the personal commitment of individuals. In order to make future missions more efficient, the aim should be to perform at least three gynecological procedures per day. The introduction of laparoscopic procedures also offers long-term potential.

Knowledge transfer through lectures
In addition to clinical work, Prof. Dr. Gnoth gave lectures on reproductive medicine and Dr. Beurer on gynecological oncology topics. Interest – especially in the topic of breast cancer – was great, but knowledge gaps also became clear. The introduction of a CME program for further training would be a sensible next step.

Objectives of the project proposal
The project proposal includes the establishment of a reproductive medicine center with structured processes, training courses and documentation of success. A laparoscopy program and structured breast diagnostics using ultrasound-guided punch biopsy are also planned.

Structural weaknesses complicate cooperation
Despite progress in terms of content, there were irritations: Several crucial meetings with authorities took place without the participation of the Eritrean project partners. This raises questions about prioritization and commitment.

The MoU as a basis for the future
The adopted memorandum of understanding forms the formal basis for further cooperation. It defines tasks, responsibilities and common goals – from further training and infrastructure provision to evaluation.

Conclusion: Great opportunities, but strategic focus needed
The trip highlighted the great potential for a sustainable medical partnership, but also the need for strategic clarification. Reproductive medicine is important, but should not be viewed in isolation. Oncological care and basic gynecological care must also remain part of the agenda. The general meeting of met4a will now decide on the next steps.
